Abstract
A 40-year-old man complained of a 4-week history of pubic pruritus, claiming protected sexual exposure 2 weeks before onset of symptoms. Skin scraping for KOH smear of plucked hairs by his previous doctor revealed no abnormality, as well as the detailed examination by his current doctors of his genital skin under a binocular head scope, except for excoriations. No maculae ceruleae or impetiginization was noted. There was no inguinal lymphadenopathy. Digital epiluminescence dermatoscopy (DED) was applied, showing a few nits near the base of pubic hairs. Further detailed dermatoscopy revealed alive and moving lice. The application of polarized light yielded definite evidence of active infestation with Pthirus pubis. Permethrin 1% cream rinse was prescribed to be applied to the pubic region, lower abdomen, thighs and buttocks, rinsed out after 10 minutes, and application to be repeated one week later. He responded well to treatment.
